With simple questions and exercises that I ask the student I quickly identify the weeknesses of the student. Then I explain the student the fundamental concepts. I make sure the student understood by having him work out more exercises and examples.
I always create a relaxed and non judgmental atmosphere and meet the student at his/her level and point out the student his/her strengths.
