When working with math students, usually on a one-on-one basis, I can quickly identify the student's strengths and weakness. From this assessment I efficiently and effectively help students understand and master concepts they are having difficulty with. I find that students are more successful when they have an overall understanding of a concept, not just memorizing steps in completing the problem. To get students to understand, I break down concepts into parts that are easily understood.
When working in other subjects, such as Social Studies and Science, the student and I have discussions about the topic. I also have the student crate a chart, table, or drawing of the topic to gain proficiency.
